Hi Hussain,
I can share with you a study material / report prepared by Ms Rashmi
( I am not sure if this is from Cite HR or elsewhere).
The report is very elaborate and well written.
It will give you an idea of a hospital functioning from a HR point of view and help you to prepare your own questionnaire.

Hi Hussain,
There is another suggestion.
Nowadays many hospitals are covered under ISO standards.
You can surf the net or even try in Cite HR archives under the caption HOSPITAL AUDIT ISO STANDARDS.

Dear Mr Hussain
I hope you have seen my second mail.
HR Audit in hospitals also can be searched in Cite HR archives and also in the internet.
From my personal experience, I can say that if you go through any ISO audit standard you can frame your own questionnaire.

Dear Hussain,
May I please know where you are studying and in which mode? The way to go about preparing a questionnaire is to search research articles on the web and study them and prepare your aim and objectives. For example I found this link http://www.worldwidejournals.com/ija...223794__93.pdf at https://www.google.co.uk/#q=HR+audit+in+hospitals
