MIS- Management Information System , regarding the Manpower status of a particular month, place and overall..
It helps the Management in decision making.. like what are the reasons of resignation.. in which area attrition is high.. and so many things to help in decision making.. with the help of MIS you may prepare a lot of reports.. Like annual attrition.. region wise attrition.. region wise manpower.. graphical representation of regions manpower status.. and so on..this is to inform what you have done at the end of the month to get to conclusion of what you have been doing the full month....basically to show the output.....whether its positive or negative....
for your reference i am attaching what i have made for the same......

i have one small doubt about the sheet what you have sent to the Aditi towards the MIS Report.
need to make changes in the calculation of the salary structure, because there is a difference of calculation in Gratuity (as per the government of india is not saying that need to deduct from the employee sal and pay back to employee after resigning from the service) this is the additional benefit from the employer to employee for the service given for about 5 years to employer.
and as per the LIC of India says that percentage should be 4.8% on basic not 4.88%.
which is incorrect and donot mentioned the word like as per the govt of laws
