The procedure you are trying to implement is good. It will work our better for any environment. However please include a third man the Dept. head or a person duly appointed by the Dept. head, let the hand over be made to him and the take over can happen from him. because new joiner may not be aware of the things what happened. Also please sit with the Dept. head and frame the points which has to be there in the check list.
This is my experience. However you can go according to your convenience and management policy.

Dear Rahul,
Please find the attached Transfer of Charge format, which we are using in our office and as Mr. Vivek said, you may include third man of the department / immediate supervisor / Head of the department, if necessary.
