Hello Snigdha Shubhangi,
WHAT has been the need for you to retain records of your activity? Has there been rupture between you & your boss? If Yes, Why? You are just a HR Trainee.....meaning too junior for any such situation to even crop-up.
Unless you give the FULL & COMPLETE picture of the situation, any suggestion....including Raghunathan's......is usually based on generalities & CAN'T be related to specific issues/situations. If you wish to have actionable suggestions, pl give the full background.

Dear Snigdha,
You have just started as a HR Trainee and you seem to be taking a confrontational route. Almost challenging the boss and trying to collect evidences !
I don't know, why you need to collect evidences, but the better thing for you would be to focus on learning and making best use of the time and the opportunity.
Give a good start to your career.

Dear Friend,
Hope you have to put a good work, expecting recognition..on both sides you want to prove good in work. Never expect your boss praise you, never expect you will be crowned immediately. Collect not any evidence let it be as it be..
1) Always have two notes, one to make what you have been instructed, another one, whether you have completed it in time or not.
2) Always keep yourself abreast of the situation, whenever you have been instructed you make a note of it, if you have been encountered, with smile you put it I want to organize, none want to intercept you.
3) In the another one you just make the works in sequence and complete it with merit.
Hope you need not any evidence for your work and for the instructions....it is simple and make your self praise....
best of luck, you are a born gem, the thing is you have to be polished ...
