Hi Marita,
You need to check following points to cover the topic.
Monthly data of people who left the organization.
Yearly comparison of the data.
The questionnaire should have :
Reasons for leaving the organization.
%age of people leaving under every reason.
cost effect of these attrition to company.
Measures to decrease the attrition.

Dear Friend.
Employee attrition is the best topic you have choosen, apart from the data above, the cost effective measures implies relieving the employee for the benefit of the organisation did not come into picture and this is retrenchment, so attrition is the better prospects of the employee for their own benefits, non-motivational reasons, promotional complaints, favouritsm, etc., the best questionnaire may contain :
1) whether leaving the job for better prospects otherwise?
2) the reasons beyond control?
3) Organisational retrenchment otherwise?
4) Organisational policies that did not suit/amiable to the employee?
5) The distance might have been cause of attrition?
6) Local unfavouring environment or the area where the organisation found not suitable for better place of work?
The minimum is the question the best will be the response...
