Yes, it is required because we have submit the form after 1st june'2015. Its not issue when you resign from Co. But in following terms Form 15 G not required.:-
• Transfer of P. F. from one account to another P. F. account.
• Termination of service due to ill health.of member, discontinuation/contraction of business by employer, completion of project or other cause beyond the control of the member.
• If P. F. Payment is less than Rs 30,000/- but the member has rendered service of less than 5 years.
• If employee withdraws P. F. after a period of 5 years of continuous service including service with former employer.

I am a bit confused on the clause of 5 years of continuous service. Here is my story :
I started working for a company in Nov 2010 and worked there till Apr 2014. I got my PF transferred to the new Company's PF account. Then I left this company in Apr 2015 for another one that I only worked for 1 month. I never made the PF transfer request but both accounts are under same UAN. I then moved to Canada in June 2015l Apr 2014. I got my PF transferred to the new Company's PF account. Then I left this company in Apr 2015 for another one that I only worked for 1 month. I never made the PF transfer request but both accounts are under same UAN. I then moved to Canada in June 2015 on Permanent Residency.

I applied for PF withdrawal just before leaving but it got rejected which I got to know later was because of not filling Form 15G. Now when the opening of first PF account was in Nov 2010(more than 5 years ago) I want to apply again, Do I need to fill in form 15G and in case I am not able to provide the form what deduction am I looking at?

Hello all friends
Yes it’s compulsory to submit the 15G Form with PF withdrawal form. If your PF account was 1 month or more then that or you resign from company any time, its compulsory to submit the 15G Form.
So I provide sample of 15G how to fill up the form.
See the attachment.
