Dear Krunal,

You have made your questionnaire unnecessarily lengthy. The recipients may not take seriously the questionnaire because of the length.

Secondly, the most important part of questionnaire i.e. business impact because of TPM is missing. Therefore, the questionnaire can be pruned down to just 5-6 questions which are very important. These could be as below:

a) % of idle manhours before TPM and after TPM

b) % of downtime of machines before TPM and after TPM (or % of availability of machines before TPM and after TPM)

c) % of scrap reduction before TPM and after TPM

d) Increase in hours of planned maintenance before TPM and after TPM

e) % of non-compliance issues before TPM and after TPM

f) Number of hours of training to the technicians before TPM and after TPM

You may change the sequence of the above questions if you wish to. The above study can be conducted for the companies where TPM is followed and where TPM is not followed.
