While you have shared the format, you have not shared the actual KRAs as such. The crux of the matter lies in design of the KRAs. Many professionals fails in this activity.
Secondly, in your format, you have given column for the self-appraisal. If there is proper evidence of the attainment of the KRA, the question of self-appraisal does not arise.

As you have mentioned, the crux of the subject is the design of the format. That is why i am sharing the format along with the evaluation process with everyone for opinion. The actual KRAs will be filled by the employees in consultation with their seniors.
I want professional opinion from professionals like you on the structure and the evaluation process.
Once i give a final shape to the format & the evaluation process i wish to roll out this. Also, i wish to conduct a training program on PMS.
Next, as regards Self appraisal, i want it to be a part of the PMS to showcase two way participation in the system and transparency to a large extent.
Therefore, please give your views on the format & the process.
