Here is d format for name change intimation ltr. You can modify it as per your needs...

Subject: Change in the name of our Company
Reference: (If any)
Sir,
We are pleased to inform you that the name of our Company now stands changed from << Old Name>> to <<New Name>>
This change is in line with << Co's>>global policy of bringing uniformity in identity of all the group companies throughout the world. (or whatever reason)
