There is no format for terminating the contract.Just issue a formal letter terminating the contract w.e.f -------(date).If the present contract has got a provision for termination,refer that also.If the contract provides for any notice period adhere to that also.

It is preferred to issue a notice stating the reason for termination of contract (if mentioned in the agreement) before issuing the termination letter.
Please find the below sample termination letter.

Dear Mr.Kumar,
Sub:Termination of contract between ABC consultancy and RTY - Reg.
Ref: Letter dated
As per the reference cited above, we have requested you to submit the details of salary disbursement of the mentioned employees made by ABC consultancy who are contracted to RTY. However, in spite of subsequent follow up in this regard, you have failed to submit the requisite details.Hence, under this circumstance, RTY have decided to terminate your contract with effect from 30/12/1243.
This is for your information and action.
