Pl visit your Factory Inpector Local Office and ask for Application Form.
I am not sure about other states but in Maharashtra State, you need to submit the following:
1) 3 Copies of Application Form
2) Building Stability Certificate
3) Memorandum of Articles of your Company
4) Company Profile, List of Raw Materials & Finished Goods
5) Production Process Flow Chart & Process Description
6) List of Directors, their DOB, Residence Address and their Contact Details
7) If your Company is in any Gram Panchayat, an NOC from Sarpanch
8) Pollution Control Board Consent Copy
9) Original Challan of Fees Paid in SBI Bank
