Management is not giving me promotions and hikes. They always rate me lower though I am very good in technology and management.
I have lots of proof of my capacity. The HR department is also not doing anything though I escalated several times at higher authorities.
What can be done?

Request your supervisor for a detailed discussion on your performance. Listen openly to what they have to say and avoid being defensive. Find out the key parameters on which your performance was judged in the interview and get your supervisor's appraisal of where you're at now. Try to know what you could have done differently so that you would have surely got promoted. Also ask your manager what you need to do in the so that you do not face rejection again.
Make sure those above you and those who decide on your promotion know that you are working hard and you are effective in what you do. Talk to people who make a difference and let them know how keen you are to keep improving, so that they know you are serious about moving up.

There is a vast difference between confidence and over confidence, when you think you are very good then these problem shouldnt come up at any point of time. There could be some reasons behind it, May be what ever you think might have looked in a different perceptive from your boss. Now every company is recognising thier employee's to retain and they know hiring and investing on them is a bigger challenge. As suggested by Ghouse, do check with boss about your KRA and check out what went wrong and what are the real expectation over you. Then you can decide whether what you have done is right or need to be improved.
On the other side, i wonder how the management is keeping quite till their employees get bugged/ de-motivated. But there should be some valid reason behind it, otherwise they would have informed you till date if any. So check with them and prove yourself in a right spirit.

Directly go to your immediate boss whom you are reporting day to day business and ask politely that what is the reason or what wrong with me so I am unable to get the change like hike & promotion. Another thing that how long youe are with this company? There may be chances that you mau be expecting change in every month? sneh

It's part of life that "Our face does not fit." or the boss is prejudiced, for whatever reason. Like Swami Parthasarathi says, "A man may be seeking divorce from his wife, but at the same time there may be another man just waiting for the divorce to get married to the same woman.*

Let me give a couple of examples that I have come across. A Dutchman once said that he was not prejudiced like the English. I asked him "If there are 4 equally qualified applicants for a job, is there anyone that you would not employ?" he replied "An Italian." That's for non-perceived prejudice. In the second case, an Asian was not given promotion, in the UK; however, he had been advised to apply for job 3 levels higher, in India. When the employee spoke to the manager, his response was "I am sure you can do that in your own culture."

If your discussing with the manager and HR does not bear fruit, your options are to quit and seek job elsewhere (There is no guarantee that your present boss does not also do the same and becomes your new boss.) or fight it out legally, if the cause is either Sexual Discrimination or religious discrimination.

Gone through above discussions, what Sneh is saying is correct without knowing the tenure of your service, intervals of appraisals. How many time you are ignored.
Second thing I would like to add which you may not like that every, there is an assessment. What we think at our own is called as self-assessment. Self-Assessment is always on higher side than the Third Party Assessment. So do not get upset with the things. Be patient. Go to your boss tell him the incidence (without any prejudice mind that he is bias with you). Illustrate him. Ask him what he want and try to restore you before the next appraisal is due.
Before concluding I would like to add here that most of the replies are with their name or nick name. BUT YOUR QUERY IS AS Anonymous. Do you feel it Auckland? Of Couse this is my thinking that there should be transparency. Here you are hiding your name also. Sorry.
