Dear Seniors,

I am working as an HR in an IT-based application development company, specializing in web and mobile applications. This startup has been running for over four years. Until now, we haven't had any standard format for appraisals. Our management has now planned to implement a format applicable to all employees. Please help me create a standard format for the Appraisal form, including both Self-Appraisal and Performance Appraisal.

You just need to use the SEARCH tool located at the top middle of the page for your needs, as there are a number of appraisal formats available. It is you who knows what you actually need, so I believe you can develop a good format or make the changes accordingly and use it.

My superior is expecting formats that are more tailored to our company, particularly IT-based questionnaires for self-appraisal. Therefore, I am seeking your assistance in creating a suitable format.
