Dear Seniors,
Please help me in below mentioned problem.
If any fatal accident take place at work place, What are the document we need to complied for Various departments to compesented his family.
Provident fund.
ESI.(How much amount he can get)
WC Policy
Kindly suggest me in this matter, as i am fresher in HR..

Provident fund will be as deposited,
Edli maximum 1.5 lakh depends upon service.
Esic pension approx.60% of gross to dependants
Funral expenses 10000.00INR
WC applicable if not cover under esic,amount depends upon age.There is chart in act for the same.

Provident Fund
1. Fill Form 10D (EPS) and Form 20 (EPF) attach Death Certificate, Address Proof, Family Details, Nominee/Legal Heir certificate, Bank Account proof. After employer signature submit the forms to PF Office to get the Pension and PF Amount to the family of the deceased.
2. EDLI : Fill the Form 5 (I.F) and send the same to PF Office so that the 20 month of EPS Contribution or Rs.130000/- whichever is less will be payable.
3. GPA Policy / WC Policy : The application for claim should be filled (which will be supplied by the Insurance company for GPA / WC Policy), attach the death certificate, certificate from employer, Compensation payment draft, acknowledgement given by Workmen Compensation Commissioner, Medical Expenditure details, Family Details/Legal Heir details, Muster Roll & Wages Sheet. Forward the same to the concerned Insurance company.
