In your appraisal process, since you have not included peers, vendors, customers etc, it cannot be 360 degree appraisal. As you have included self-appraisal, at best it can be 180 degree appraisal and not more than that.

if one person rates that is 90 degree if two person rates that is 180 degree if three person rates that is 270 degree if four person rates that is 360 degree

It is not just number of persons. Their level also matter i.e. whether junior, senior or peer. Please take into account their designations also.
