Greetings All!!
The name of the PF account holder is wrong in EPF records and now the PF account holder is deceased and the claim has to be submitted by his wife.My doubt is shall we fill the Form 20 D with correct name as in ID cards or shall we fill the form with the name in EPF records and provide the affidavit for the correct name and relationship with deceased.

I think its 10- D not 20-D
hear there are 2 types of precesses you can fallow
one:
u can fill the From10-D as per PF records and get sign form the PF member (if he can't sign there is an another process) and get the PF amount released. the bank account name should bit simile to the PF records name otherwise money wont get transferred.
second way:
you can meet company HR and apply for name correction on the PF records, The company HR will write a letter to PF officials and with that letter he will submit the past Returns and other related documents u have to submit the true name prof with some process the Company HR will do that
after that you can claim 10-D.
This will take some time.
you choose which one is suitable to you

As such the name of the deceased is wrong in EPF record, pls approch your employer to fill up Form 20 and Form 10D and all other forms as required for PF withdrawal with a clerification from the correct name of the deceased, an undertaking from the dependent of the deceased is also required.

Obtain a letter from the wife of the deceased requesting correction in the name and forward the letter to .P.F department along with your covering letter endorsing the correction on the basis of company's records. Should need arise enclose copy of bank pass book of the deceased as evidence.

You have to fill form 20 for EPF claim and form 10-D for pension claim. the nominee should produce a Legal Hire certificate given by thasildar mentioning spouse and the children name who are all under 18 (male) and if female 25. bank accounts should be opened seperately for all. then the claim forms can be send for process

Dear Singh,
If you know that PF member name is wrong in PF office record then you can produce a affidavit other wise you can produce his Date of Birth certificate from school or MC.and also mention in your covering letter that due to oversight we had given wrong name.
