Dear Ms. Indu,
1] Induction is a initiation bringing on the new joinees on board to explicate the History, Culture, Organization chart, Processes, HOD's etc. Likewise, Checksheet is a structured form that is prepared for collecting and analyzing data; a generic tool that can be adapted for a wide variety of purposes used in Quality Control.
Checklist is to prepare a list of items that needs to be collected from a new joinee (set of documents) produced at the time of joining formalities done by the HR.
2] There are many self assessment forms posted in this forum earlier and requet you to use the "Research" facility at the top to get the deired as per your requisite. https://www.citehr.com/19729-self-as...rspective.html and <link outdated-removed> is for your ready reference.
No any format has password locks except if the member wants to lock the document voluntarily, usually such documents are rare at cite. Please pursue more documents which are already in use by members of this Cite Community.

Maybe your boss is referring to an Induction Checklist? If so, then it is the same thing. An Induction Checklist basically means the different events (as mentioned by others) that needs to be carried out as part of an Induction process. For every event completed, both new staff and the person responsible for that specific event needs to acknowledge. Once the whole process is completed, a final signature and form is submitted to HR for filing into personal file of employee.
Separately, if you do not understand or disagree with you boss, it is better to get further clarifications from the horse's mouth then do the guessing game....that's what communication is about.
