Dear all,
Please find attached format of Affidavit to use in case if any one lost their PF scheme certificate.
For issuance of duplicate scheme certificate in the event of loss of original scheme certificate, a specific request is required from the person or legal heirs (Beneficiaries) and the said request shall be accompanies by an affidavit cum indemnity bond executed in Rs. 20/- non judicial stamp paper duly attested by 1st class Magistrate/Notary Public with signatures of two securities indicating their names and address.

I have lost the original scheme certificate which was issued from my previous employment . Could you please let me know the procedure to get the duplicate one ? .

My experience is less than 10 years (which is approx 9.5 years ) and the current organisation did not accept my previous scheme certificate claiming that there is no provision as per the government norms . the only option suggested is to keep the scheme certificate with me till the age of 58 years . Kindly advise
