Dear Poornima,
Thank you for posting the questionnaire. Here are some critical comments and suggestions to help you.
Please edit your questionnaire to avoid typos; for example, see Q25 "25) Do any of the following hinder you in balance your work and family commitments?"
Asking personal and demographic questions at the outset may put some respondents off. Hence, it is better to move them to the end.
You have not said why you are doing the survey and what you will do with the findings. Only polar (Yes/No) questions are not suitable for degree level projects. Please read some material available at CiteHR by clicking on the links in the sidebar at the right.
Finally, kindly use a web-based survey if you want to improve the response rates. Expecting people to download your questionnaire, answer, save and send it back to you is not on.

Dear Sandeep,
Thank you for your comment. Normally, I send Private messages; but as you are a new member, I could not.
Also, to stop members just posting one line comments, I would like to ask how such a comment adds any value to the thread. Did you read my comment above yours before commenting?

Good day, I am doing research in the area of work-life balance. Can I download the questionnaire as well or do I need some sort of written permission for the relevant persons?

Dear Pinhead, You not only need to get permission from the author, which (s)he will gladly give you. You also need to cite in your dissertation from where you got it, etc.
