Fill the joint Declaration form both Employee and Employer should duly singned and submitt to nearest dispencery it will change 3-4 working days. Regards, Chethan

Just visit to your local/branch office of ESIC. and filled a written request letter in your letter head along with employees correct name & existing name, their respective ESIC IP no & govt id proof.
If possible then ESIC branch manager will be amended their name in esic portal same day.
