Dear All,

On Sunday, 22.04.12, there was a major explosion at the Mitsui Chemicals Plant in Japan. You may search for details of the accident. The cause is not yet known. Attached is the Webstac bulletin that also addresses the issue. Let us try to keep track of the root cause analysis and actions being taken by this company and learn from this incident.

Thanks & Regards,
Sudhir

Dear Sir,

I also read this news yesterday. I did not upload the same as nowhere was the root cause and recommendations mentioned. Are these Webstac magazines you are receiving on a chargeable basis?

Thanks and keep on sharing.

Dear Dipil,

These are free for me. We are their customers. We purchased software from them for EHS legal compliance, which is excellent. I do not want to market their products. However, their conception and the actual benefits of using the software are very good.

Thanks & Regards,
Sudhir

Dear Sir,

Thanks for the information. In another thread started by you with the name Webstac Bulletins, I had a good discussion based on the news in the latest uploaded bulletin. I will share the same by tonight. Keep on sharing and keep up the pace.

Dear Mr. Sudhir,

Thank you for the information. It is very sad to have such explosions across the globe, despite having established safety systems. There has to be some violation, probably in procedures (shortcuts), corresponding to human error. Deaths and the simultaneous loss of several dollars cannot be acceptable or justified. For such organizations, operational disciplines have to be followed regularly. Plant management has to ensure that operational discipline is in place within the organization, inclusive of the leaders of the organization.

I hope there is no significant or uncontrolled damage to the site or the surrounding community.

Regards,
Sanjay Ghiya

Dear All,

All the health and safety professionals should read the attached bulletin. It gives news from leading newspapers in India, and we can think of what could have gone wrong. We can then try to identify similar hazards in our companies and, if so, try to implement the controls so that similar accidents do not take place at our workplace. Attached is the latest bulletin from M/s Webstac.

Thanks & Regards,
Sudhir
