Hi, Plz share working of 360 degree performance appraisal system.
First time we have decided to go with 360 PMS and me and mine team have gone through theory and other related facts n Figure. At present we require Form and other documents that shd be circulate among team members to actually start implementing 360 PMS.

360 degree PA system basically involves inputs from different verticals which are important to the organisation, ie peers, customer, superior, subordinate etc.
So while creating the appraisal form, i think the company culture and work environment forms an important contributor to the success of the whole 360 degree PA.. So it would be beneficial for you, if u make the form yourself, rather than using the already available ones in this forums. Coz a slightest mistake in the form , that might act as a resistance to the organizational change in your company.As a result the peers , subordinates might not appraise the individual in transparent manner.
