Nice questionnaire; I have a few suggestions.
1. It is better to ask demographic and personal questions at the end.
2. In Part C guidelines you have missed out indication what "D' stands for. I first thought the scale was skewed, but found it to be ok when I saw the table.
3. To improve the response rate, you need to use a web-based survey, downlaoding the questionnaire, answering it, and then sending it to you is time consuming.
