Hi! I handle HR in a ITES start-up. I have been working in the OD area and therefore don't know much about the nuts and bolts of HR operations. We currently have 14 employees. Please could you help me by giving the list of mandatory documents that need to be maintained by HR. Also we have the policies available on our intratnet - do we need to give them paper handouts in the form of an employee manual.

Dear Tracy,

The list is as below:

a) Proof Date of Birth

b) Proof of Address

c) Xerox copy of the certificate of highest qualification he/she has attained (candidate is to be told to bring original copy also. HR Head or Executive is to verify the original with photocopy and attest it).

d) Postcard size photograph with family members – 2 copies (applicable only for those who are covered under ESI Act, 1948).

In addition to the above, following forms are to be filled on the very first day of employment:

e) Employee History Sheet (I have uploaded already, you will find it in this site)

f) Form 25 (Form of Nomination as per Rule 125 of Payment of Wages Act 1938)

g) Form F (Form of Nomination as per sub-rule (1) of rule 6 of Payment of Gratuity Act 1972)

h) Form 2* (For Nomination and Declaration Form as per PF Act, 1952)

f) Form 1* (Declaration Form as per Regulation 11 and 12 of ESI Act, 1948) (applicable to those who are covered under ESI Act and that too for those who do not have ESI Card)

i) employee must give address/telephone of the next of kin in case of emergency.
