Dear Santosh,
Following are the FORMS which you need to be submitted for PF in Case of Death of an employee: -
FORM 20,
FORM 5IF,
FORM SFC,
FORM 10D,
FORM 5, and 10,
FORM 3A,
Original Death Certificate,
Nationalised Bank Account of the Nominee
Passport Size Photos

Form No. 20- for Pf withdrawal by Nominee, Form 10 D- for benefit of pension of wife/children up to 2 No.
EDLI: for insurance benefit under EDLI scheme Form 5I F.
Documents required: Death certificate issued by District Authorities. If nomination is not done by the employee then forms should be supported by affidavit of nominee/legal heirs. Bank saving pass book of wife and eligible children (of same branch of SBI/Punjab National Bank or any bank authorized for pension purpose by PF authorities, 3 sets of passport size photographs of each eligible member under pension scheme, family detail on company letter head, Date of birth certificate (in case of children issued by the school authorities), Certificate of company declaring that employee was in service of the company.

dear all
one of our employee left one year back after the 8 months of service, their after he expired with out withdrawl his provident fund he was unmarried at the time of death.
can anybody help me can his nominee entilte for penion or not.

arun kumar is right but here some more thing also need
1. birth certificate of died person cause the pf authority want to confirm that the died person was below 58 years when he died.
2. ration card copy of died person`s wife and children.
3. birth certificate of died person`s children.
4. bank acoount no. of died peson`s wife and children separtly in state bank of india or punjab national bank or hdfc bank ltd.

Dear Santosh,

In death case you need to filed following forms with supporting documents.

1) Form 20

2) Form 5 IF

3) Form 10 D in Three set

4) Form 5 & 10

5) Death Certificate

6) Date of birth certificate for each claimant including member also

7) Bank Account for each member (in case of minor under gardianship A/C) for this purpose bank account will be opened in specified bank (SBI, UTI axis bank, Punjab National Bank, HDFC Bank)

8) Passport size photo 3 copy for each member

9) Family photograph of all claimants

Note: In case of Minor claimant the major employee will signed his/ her Form 10D on behalf of Minor

Form 3 A for the respective period

Enrollment certificate form 5 IF in below format

Certified that the member died on ____________________________while in service.

Certified that the provident fund accumulations of the deceased employee Late Shri/Smt.__________________________________A/c. No.__________________ were paid to Shri/Smt./Kumari ___________________________
