I was working with a BPO from May 2009 till Mar 2011. I got terminated from that company. I am trying to withdraw the PF amount on my own, but at the EPF office they are asking for form 3A from the employer. Can I download the form and fill it myself? If yes i need your help in that. Also what should i do for the attestation in that form. Please help on this.

You can not download the form 3A. The form 3A is the statement of your monthly contribution details which should be submitted to the PF authority from the Employer. Kindly contact your principle employer and ask them to send it from their side
