In the case of death of the employee, his/her spouse should apply for payment of PF dues, deposit linked insurance as well as widow/widower pension which is admissible with two children upto the age of 25. For pension, form 10-D is to be filled in. Death certificate of the deceased employee is must. Photographs of the applicants, bank details etc. are required. For children, proof of dates of birth is essential. In the case of accident, the documents change. Please specify whether the case you refer to is of death or of accident. Thanks & regards,

8 Family member certificate (availabe at PF office)
9 Form 5,10 & NRS
10 Form 3A for the current and previous yr. attested by principal employer

Follow what shyam and sanjeev have suggested they r correct in addition u have to attache a list of surviving members of the deceased member and also if was in the services on the date of death then issue a death while in service certificate alongwith form 10 D.
