Are you a student intern? When will your internship end? The best source for ppt is the web. The best way to go about is to adapt what you find on the web, design your own and then seek feedback on that. I do not work in that industry, and that's what I would do if I wanted something urgently.

Dear Sushant,
What Mr. Simhan stated is right. You just look at right corner, you will find no. of PPTs and material on Performance appraisal to help you.
I have not worked in service industry, but just as a clue can tell you, while doing appraisal, you have to think about two factors - `goals' or `targets' given and behavioural attributes like team spirit, interpersonal skills, cooperation, delegation, quality of job, discipline and so on.....as suited to your nature of organisation.

after searching a lot on this site ,i have got some information about the ph.d. but i am still confused about the process as how to get started.
what steps i should to take for ph.d means how to select the topic, reseach methodology etc in the field of HR.

The best way to go about is to find the gap in knowledge. To do that you have to read articles in scholarly journals and choose a topic that interests you and that offers an opportunity to add something new. Usually, authors write what further needs to be done in their articles. Please access Emerald and read abstracts in journals in your field.
