Hi Ronnie,
I think you need to clarify your question a bit because it is very broad. Why are you doing the competency assessment? Is it for selection, career development, etc.? Are you looking for a paper based method or a software based method?
My company does competency assessments. We have a software product that helps employees to identify their skills gaps. Then the product allows people to create their own Individual Development Plans which link learning resources to their skills gaps. Before the product can be used, a job analysis must be conducted in order to identify the needed competencies for the job.
