1) name of the service provider
2) whether individual term insurance or group term insurance
3) coverage limit per employee
4) whether company sponsored term insurance or employee also need to bear partial premium cost ?
5) coverage starting date/month.

The name of term insurance provider is____ and the coverage limit per employee will be____. Please note that being group term insurance individual policy copy will not be issued to employees. The term insurance coverage is applicable to employees while working with our company only.
As a welfare measure Company is paying the total premium to the service provider and it is a complimentary term insurance for all our employees.
Employees who are yet to submit nominee details are requested to contact HR dept to update your nominee details.
