Mistakes are great learning opportunities and one does learn from it. However, one can not talk about mistakes in isolation - they have consequences! Some body can die - or some one can incur heavy losses. Fires caused by operators not following safety norms, driving mistakes on road etc. etc. are to be avoided. Mistakes will continue to happen - but there has to be mechanisms in place to make sure they are captured in time before the consequence occurs. Bcoz of hurt, someone again does same mistake as its not happen with me.
