Dear Akta,
I thing You need to change scale for measuring of performance because you have mention both type of questions ( like Positive and Negative)

This is not easy to evaluate in X & Y

Yes Faruk you are right but, I feel we can not keep all only positive or only negative.
For appraisal we need to check out both the sides. C
Can we assign weight to different aspects.
And is the format proper or it should be in MS word format?
