What roles are these joinees working on? Was any goal plan discussed and documented when they were inducted? How would you ensure, that they were aware of the parameters for performance right from the beginning? I am sure you took orientation session. Have you documented all that you shared with them and got their signatures in it?

I appreciate you designing and sharing the form. I did find few areas missing, there was no productivity score card and area to measure abesentism. The areas that you have covered are largely behavioural . How do score them on productivity and efficiency ?
