on top of what Phanish has said, I would like to add that 'change management' usually entail aligning organizational culture and people to incline to introduced or prospective change either in organizational operating strategy or business approach, alteration in organizational or business structure and/or modification/transformation in organizational systems.

Change management is a process to proactively manage the people side of change to achieve the desired business results.
