For conducting a telephonic interview, you must first ask the candidate if its a suitable time to speak to him/her.
If not, take the best time that you can make him a call.
To start with the interview, you can ask the following questions: (Telephonic interview is just the basic information you take from the candidate)
1) Present Job Profile and Company Name
2) Current Salary
3) Expected Salary if selected
4) Notice period if any
5) Reason for quitting the present/Last Job.
6) Best time to come for the interview -

Say No,ask him when should i get back to you. if YES, tell them
this is regarding to sound you an opportunity with one of our client ....(Mention client Name)...give details of what skill they are looking for.
Are you looking for a change?
Yes proceed.
Present CTC
Expected CTC .....( is it negotiable)
Notice period.....(Is it negotiable)
reason for change .....ask him.

Well, everybody has given you very valuable inputs. However, one thing has been left out. While taking a telephonic interview, its not only important to ask him/her about Current Organisation, salary, etc, but also its important to ask about his/her current designation and the job responsibilities. Then only you will understand if he/she is suitable for the job profile you want to hire him/her for.
