As requested by you , I am attaching the P.F Re-authorisation form. Please note that you have to change the address of the form on top according to your area P.F office, because this form is based on Mumbai. Apart from this, we can use the same across the India.

I have been asked to submit the re-authorization form for the as name in bank is not expanded and in my of account it is expanded.
I googled the form and got it your website. If its the right form can you please guide me in
1) What details need to fill in reference section (e.g. intimation No.)
2) what need to be fiilled in the sentence "AG under copy to the undersigned".
Also let me know if you know long does it take to refund after Re-authorization process. Awaiting your response.

Even I am facing an issue more or less same as above. I have submitted the PF form and now it is under process. The bank A/c I have referred has got inactivated 2 weeks back. Now I want to give the different bank A/C. All my PF details are maintained in Mumbai office (Employer and PF office).
In this case can I send the reauthorization form through courier to my old employer asking them to attest and can I send that back to PF office? Does that works?

#AnonymousHai, Need Help about PF with drawl. i left my job in Chennai on 2016 and i joined in Bangalore. previously PF was linked with axis bank which is my salary account. Now my salary account was totally closed. i have new account with CITIBANK and details are totally different like mobile# and name and address details with previous company and PF office. can i change my details in PF office . I need to get that PF amount to my new active account. How can i change details with PF office.

1. The sample form applies to Maharashtra region only.
So For other regions, can one write the text in a document with appropriate details, take print out, get ex-employer signature?
2. If ex-employer is not available whose signatures can one take?
3. Does one need to visit regional EPFO office or one can send it by courier/registered post?
