Please search for SERVQUAL, SERVPERF, LIBQUAL, etc. What do you mean by "service delivery at university"? What service are you condisering for your study? Are you considering the service provided to staff (academic or non-academic) or students, or outside bodies?

I mean service provided to students by the university such as 1) teaching and learning 2. library services 3) catering and accomodation 4. IT services

They are all different services and you cannot use a single questionnaire for that. As i suggested please search the Web. You need to study about SERVQUAL concept popularised by Dr. Prasuraman, et. al., and digest it.
I have used that concept to improve Learning by taking student's views about teaching sessions.
