Subject: How to Fill Certain Columns in Form 19 and 10C

Hi Sir,

I worked for a company from 2007 and was relieved in March 2009 in Bangalore. I am currently pursuing higher studies. I want to withdraw my PF money now, but my ex-employer is not providing assistance or responding to my queries. I have downloaded forms 19 & 10C and plan to get them attested by the Bank Manager. I seek clarification regarding certain columns in Form 19 and 10C.

1. Form 19
Contribution for the current financial year:
Problem: What should I enter in the table as I resigned 2 years ago in March 2009?

2. Form 19 & 10C
Name & Address of the Establishment in which the member was last employed:
Problem: My ex-company relocated to a new address after my resignation. However, I worked at the old address. Which address should I use in the specified column as I am confused about that?

3. Form 10C
Are you willing to accept Scheme Certificate in lieu of withdrawal benefits?
Problem: Should I respond with "Yes" or "No"?

Please assist me!

In the current financial year, the contribution is NIL. To support this, you should attach the service certificate, experience certificate, or relieving letter issued by your previous employer.

In the address column, mention the company's name. Whatever address is stated in your service certificate can also be listed. A separate sheet can be used to inform about the shifting of office premises to a new location. Additionally, ensure that your establishment code and PF Number are provided to display all necessary details. There is no need to worry excessively about this.

Regarding the acceptance of a Certificate in lieu of money, if you answer yes, they will issue a certificate. However, if you select No, they will pay 8.33% of the emoluments available in your account.

You can directly submit the forms to the concerned PF Commissioner's office along with the Bank Pass Book's first page, proof of name and address, and a copy of the service certificate.

Do not forget to include your Mobile Number at the top of the application so that you receive an SMS alert about the cheque release and deposit details in the bank by EPFO.

In which part of the Form 19 (2 pages) & Form 10C (4 pages) should I get the attestation as some bank managers are doubtful? Or should I get attestation on all the pages of Form 19 (2 pages) & Form 10C (4 pages)?

Can I send the application through registered post (with acknowledgment) or should I directly submit it to the Bangalore PF office as I am in another state?
