Your questionaire is excellent and perfect. I have read it all. Even i will think more and will definately suggest if some good point strikes in my mind.
Almost all organization rely on the suggestion scheme which is really successful, and the rewards, most of the time are monetory, but it should also include appreciation which we use by highlighting as the best suggestion of the month on our various notice display boards and in our regular journals.

Also there is clarification required on how can an organisation measure the effectiveness of such reward and recognition practices??
Is it through REDUCED RETENTION,IMPROVED PRODUCTIVITY??
And if yes...how do we justify that its because of these practices and not other extraneous factors, as its difficult to separte one practise from another initiatives taken by the organisation to make itself more agile.
can we assign %age contribution of reward and recognition practices to the inresed effectiveness..if yes, how much???
