It has been brought to the notice of the management through your _______that on __________you behaved very rudely with your _________. On the above-mentioned day, while on shift, ________ noticed that you were wasting your time instead of processing documents. As a Team Leader he has full right to ask you to get back to work, which as a responsible supervisor, he did, but instead of apologizing and doing your work sincerely, you tried to intimidate him by yelling at him and speaking very rudely. _________

You are fully aware of the code of conduct, which you need to follow on the floor and no individual has the right to speak to his/her supervisor in this manner. Even in the past you have been verbally warned for _______________________________.

However, even though you have received numerous verbal warnings, you have not shown any sign of improvement in behavior and attitude, which is totally unacceptable and amounts to insubordination and display of unprofessional behavior.

You are aware that such acts are completely against the company rules and policies. However, since you have apologized and promised to show improvement in your behavior, with a view to give you an opportunity to improve, the management hereby warns you to desist from all such acts. Any occurrence of such acts in future could lead to termination of your services.

We hope that better counsel prevails upon you.

This is in reference to your personal discussion with our Executive – HR, dated ....... seeking your explanation regarding your behaviour with seniors and handling Company property which was considered Un-professional Behaviour. During the discussion you were explained and guided to follow the norms and hierarchy of the Company. You were also informed about the expectations from you as an employee and a Senior member of the team.

The Company has provided you with opportunities to bring change in yourself in terms of your behaviour, attitude and your work performance. You have been constantly guided and counseled for the same. You have also been provided an opportunity to act as Lead in a Critical Project. However, it is observed that there has been no positive change in the same.

Please note that your commission of any further act of misconduct will invite disciplinary action, which may include your termination from the Company’s service as per the discretion of the Management.
