My boss has also told me to make a questionnaire type of appraisal form for the managers.
Which i have made but he is still not satisfied with it. He want me to add more questions to it, where we can actually evaluate them. If someone can add ur suggessions to it, will be really appreciated.
Few questions which i have entered in that format are:
Define ur KRAs?
How was ur past year for u and why?
What was ur most important acheivement?
Mention the most difficult element of ur job?
Elements of ur job that interests u most?
How u take ur criticism? Explain with an example?
How many leave u have taken in past 6 months?
Do u take suggestions from ur subordinates? Recent suggesstion u have taken and where have u applied it?
Which was ur most successful presentation in past 6 months? and why?
How do u coach and develop ur subordinates? Explain with example?
State ur recent team building exercise?
State ur recent strategies and directions u have taken in favor of company or employees?
What are ur goals for next year?
What support u expect from ur boss to improve ur performance in ur current position?
Areas where u feel u require training and why?

Have you done any literature survey and formed your aim and objectives for the research? You also need a conceptual framework for that. Have you any hopotheses to test? If so, could you please state that so that you can get a focussed answer.
If you have not done any of the above essential preparation for conducting a project, I would advise you to please do so and then pose a precise question.
