I am very much thankful if you could produce me details with a clear perspective regarding ESI as there is no proper process of ESI nomination form submission and number allotment to the employees.

Kindly let me know on following aspects

1.How to submit ESI nomination form and what are the enclosures employer need to submit for ESI office along with the form(ex: family photo, parent declaration form etc.,)

2. After submission of forms what is the time limit for getting ESI cards?

3. Suggest me if there are any new procedures evolved for ESI

4.Can anybody explain about Smart card for ESI?????

5. After getting card how can we get medical benefit and whom we have to approach for proper treatment

6. How can we know on what type of medical benefits can employee avail?????

7. Suggest me which website is good to know the latest updates on ESI???
