Can the industry succeed in accident prevention through BBS? Let me bring my thoughts as like below on this statement:

Only through BBS, Industry cannot succeed in Accident Prevention. BBS should be one of many such practices for Accident Prevention.

Request you to go through the first slide in the attachment and let us know your thoughts on what possible practice / tool / system available in today's industrial scenario to replace the BBS here?

Now I am totally agreeing to you on the statement of Safety is always Behaviour Based. As you said we can’t access/measure the personality / attitude of a person. We can observe only the outcome of these, ie. Behaviour. As like in Domino Theory pointing out , fault of a person, as the reason of accidents, this is one the biggest reason of failure of BBS in many places. People taking or considering BBS as a fault finding tool and tool for disciplinary action, leads to total failure of it.

Everyone just blame the people even after happening an accident and try to escape. Whereas in fact, behind an accident of any kind, even small, there must be three causes broadly classified into System, Physical and human cause. So personal fault is there, which is of no doubt. Please have a look into the attached slide no . 2. Which says 96% of accidents are causing by Unsafe Act. It says that the remaining 4% conditions, which is beyond human control and which are like natural distints.

I still remember in one of our earlier discussion, you brings out an attachment showing pictures of unsafe conditions. (https://www.citehr.com/230573-behavi...back-pain.html). Is those really unsafe conditions which naturally occurs. No. There is a person behind creating all such unsafe conditions. Hence we called it as Unsafe Condition created by Human and broadly classified as unsafe act. So in this case as 96% of the accidents causing by Human intervention, the need of controlling such unsafe act / at risk behaviours at work is of utmost necessity. So if not BBS, how we can achieve this? What tool we can use to make sure people behave in the manner they suppose to, at the work place, so as to reduce work place accidents?

Even though you pointed out that in the well structured Army forces accidents do occur. So how we can achieve to the ultimate goal of succeeding accident free workplace?

This has reference to the posting from Mr.Dipil.

Research in science is never ending. Safety is an art and science. Research in the field of accident prevention goes on and it will continue till end of mankind. However; unless all the human population is from cloning and grown in controlled environment personality of individuals will differ, and hence behavior will differ and accident will continue to occur. We can succeed only in limiting the number of accidents and not totally preventing it. If you tell this to safety people who stand for zero accident you will have to stand alone like the man who first found the earth is round.

All safety practitioners go through various writings and research findings available. They will follow certain theory which they temporarily feel acceptable. When they come across another theory they will consider it more acceptable and fall for it. They start preaching it. In fact how many of the safety professionals have consciously developed their own thoughts.

The two slides you produced:

They have no authenticity. There is something called defense mechanism in personality. Fantasizing is one. BBS preventing 100 % of possible accident is someone’s fantasy.

There are very many causes contributing to an accident. Few are evident and many are hidden. We take few acceptable causes and declare them the real cause and cheat the society. Poor society lives with it.

My request to all safety practitioners is to read all the material available and start thinking of your own. There is a vast area yet to be discovered in our profession.

ACCIDENT - an undesired event that results in personal injury or property damage.
INCIDENT - an unplanned, undesired event that adversely affects completion of a task.
NEAR MISS - incidents where no property was damaged and no personal injury sustained, but where, given a slight shift in time or position, damage and/or injury easily could have occurred.

I do agree that Zero Accident goal is an imaginary state...
Can we take this discussion to the next level beyond BBS. What the best way to reduce as much as possible incident at workplace. Which is the best tool available as of today which can help in affecting the Attitudes/Personality of a person and thereby he/she start behaving in the desired manner at work place.
Say an example, if there is an SOP of grinding job. It says that, one who is doing job is to use Eye Protection and Hand Gloves. All the employees before deputing to this job, given training on this SOP. Still during work, some people found not using eye protection and hand gloves. This is an unsafe action having potential to leads to accident.
How we can control such kind of unsafe behaviour at work?

All may agree on statement that " No one comes to factory to make themselves injured " so We need to think that -
- Why some people found with proper PPE ( what was positive force ( activator , consequences ) there for safe behaviors?) so that one can concentrate on that.
- Are PPE is as per intended purpose and accepted by user ?
- Have we discussed with user who were not using PPE ? What is there feedback
- Based on their feedback what is our line of action ( Training , signage display , Change in PPE , Availability issues, ETC)
I hope other safety professional /HR professionals will participate in this great discussion and share their experiences .

When will a safety man be of himself?

Like any other professional most safety professionals bring in gathered information from various sources and forget for once that we can exam this information critically. Again we find that safety people always think of safety only in an industrial perspective.

1. Why not we discuss accident in a universal perspective?

2. Why not draw our own conclusions and come out with ideas we feel fit.

In any accident industrial or otherwise we have three elements to consider: Human, Energy and environment. Hazard generation mechanism is based on this.

Man has to satisfy his needs; either primary or secondary. His drive towards a goal is need based. His motive is just to satisfy his need. In this regard everyone is motivated. There is an interaction between him, energy and environment. There are very many items to consider in each of these elements. How many of these items are identified and analyzed for accident prevention. I am sure not all !

For once consider environment. In a hygienist’s viewpoints thermal, light, ventilation, radiation and the like are considered and dealt with. This is the environment that may help physical comfort. But another serious issue that cannot be resolved by him affects the person – psychological.

Consider a family where two brothers are brought up by the same parents. Do you consider the boys have same environment? Do you consider them to behave more or less in the same manner in a given situations? Answer is no, for they have different environment. It is so simple. Here the elder brother has a younger brother and he does not have an elder brother. In the case of younger brother, he has an elder brother and no younger brother. Such simple a thought when added to accident prevention its scope is enlarged. Some may consider this a simple issue. However when considering behavior of individuals it is essential to understand how his behavior is influenced by the environment.

In my presentation I have used many slides mostly pictures. A few are attached here.

As a baby there is nothing to worry for his/her safety is in the hands of its mother. As a baby grows to childhood slowly the blanket protection is slowly removed. One has to be of himself/herself. Here the safety awareness created by the parents, teachers and the society goes a long way.

In most cases parental input was insufficient, there is nothing in the school syllabus to help, and alas society gives his a negative impact. The safety culture desired is not inculcated to cause a behavior based safety.

So, where we have to start with BBS?

My suggestion is from homes, from schools and the society. Through these agencies a citizen’s safety culture is molded and it will reflect in not only industry but in public too.

Great to go through the slides... The way you expressing in it really attractive to children's and easily understandable manner as well... Pretty great way of teaching...

I do agree that the information I bring forwarded was gathered one and without much detailed analysis as you done, I have presented the same to this forum... As you mentioned in the slide attached the inquisitiveness nature in children's can cause accident, this nature of mine leads to learn a lot beautiful ideas from you from this posting... So I am not having any regret in bringing those... However will try to the best of my ability to analyze the raw data getting, before submitting to such forum...

I read many debates in internet on the effectiveness of BBS... This was the simplest way of explaining why BBS can fail in any industrial atmosphere... As like you said, the need of a person can be the reason of causing an accident, the question of what is the best proven accident prevention tool in the industry is irrelevant.

We can make people educate.... Training and competency building is one of the vital part of accident prevention... As like a kid touching on the burning candle, even after make him aware 1000 times, it can burn your hand, until he experience the burn he never stops... I really would like to understand the controlling of such behaviors at work... The kid doing because his unawareness.. How we can dealt with the conscious unsafe behaviors at work?

Take an example... No mobile use zone... The operator still going inside with mobile phone??? How we can control this behavior? Supervision? Policing? Punishment? Motivation? Education/Training already given and he is one of the most competent staff... The reason or say need of using phone may be; 1. His confident in not causing accident if using mobile in the area, 2. Some personal issue at family which make him compel to attend the call at the same time to enter into this restricted area?

I am bringing this to just to get to know your views as well the views of other forum members only... This is just to explore and to understand more ways on which how we can make our work place safer...

I have a different version backed by the definition given in the the latest international standard ISO 45001:2018 which has replaced the previous OHSAS 18001: 2007 std. on Occupational Health & Safety Management System. Refer below.....
Section 3 - Terms and definitions (3.5 precisely) of this std. which gives definition of the Incident, Accident & Near-miss
Incident
occurrence arising out of, or in the course of, work that could or does result in injury and ill health (3.18)
Note 1 to entry: An incident where injury and ill health occurs is sometimes referred to as an “accident”.
Note 2 to entry: An incident where no injury and ill health occurs, but has the potential to do so, may be referred
to as a “near-miss”, “near-hit” or “close call”.
Hope this clarifies the diff.
