I need your help for PF Transfer Issue.
When I left my previous company, I had asked to withdraw PF to my bank's account. However, due to some operational reasons, the account was closed by the bank.
I had gone through the site EPFO <link updated to site home> on this point 22) discuss same thing and they have suggesting to submit application stating about such problem.
I had discussion with that company HR about this, they are ready to give such latter but they are asking for "proforma".
I have no idea about what "proforma" is!!
Can any one pleas let me know about "proforma" or give me sample "proforma".

Dear YOgesh,
What you have to do is to submit a reauthorisation form giving the correctd account number and you will get your cheque. This is a common thing which occurs in software people who ignorantly give the salary account number which ceases once they stop drawing salary.It is high time youngsters started using nationalised banks savings accounts on the long run as they are good and do not close unnecessarily the accounts.
This reauthorize form is available on the epfo web page. if not you could ask for it in reply and i could try and attach the same.
