Please forward me questions for 360 degree appraisal and also make me clear that ther are diff qeustions for diff appraisers means we need to frame diff quests for peers, subordinates, supervisors, customers or same quest for all of them????

Hi Poonam,
You may have different questionnaire formats for the different categories of respondents or same questions for all - both practices are acceptable. Usually the second approach is followed where the respondents mark the questions applicable to them and leave the rest.
Also the data analysis is easier in the second approach.
