I want to conduct performance review of our employees and need your asisatance to design self appraisal tool for the category of staff and executvie.
Would any body can guide me that what sort of parameters or attributes should i keep while designing self appraisal tool?

Do u refer any handy competency dictionary or you define on your own the 5 levels. And one more query, the requirement of this competencies may not be same at all level, so do u prefer to first define the job pattern and then rate each competency.

I would also like to know when the appraisal has to be conducted.R there any specific months for conducting appraisal.What is the score a person has to get out of 5 so that he/she gets promotion?
What is the % of hike given generally in most of the companies?
Please give me some inputs. :idea: ....
