Dear Mantu
Format on Salary certificate is not provided in any of the Labour Law but it is clarified that rate of wages is written in the Service Certificate to be issued by the contractor under the Contract Labour (R&A) Act & Rules. You may also take the copy wages slip issued by the Contractor under the Minimum Wages Act & Rules in Form XI.Even if the insurance Officer is not satisfied then ask him to give the format for issue of the salary certificate otherwise contractor be asked to issue a simple salary certificate.

Dear Anushka,
You can utilize your own sample salary certificate. In place of basic salary you are to write consolidated salary. Even though it will be better to inquire from the bank itself for the draft of salary certificate. You will have to approach the bank for loan then you can also ask for this formality along with other formalities.
