TO begin with, ISO 9001 standard does not dictate the method of training, but it says, identify your compentency needs, evaluate the gaps, thus identify training needs. Then plan, train, evaluate, review the training process, as any other process (PDCA cycle).
How could anyone in this forum tell you about the competency matrix required for your organization? It depends on the Scope, nature, complexity of your organization's activities.
Look at these requirements in the context of the available competencies of the team (top to bottom).
Coming to the next item, FORMATS. ISO 9001 standards do not dictate any forms/formats.
Please go through the "process flow" of your organization and ascertain at which juncture of the process, you could introduce / improve the existing record keeping practices of your organization. (For reference to the manadatory records required for the standard see the attached document)
