Submission Of ESI Half Yearly Returns

Pls. clarify whether we need to submit Form 5 in Quadruplicate or one copy to ESI Office as per the latest amendments in ESI Act.
Appreciate if I can get a reply immediately.

Form 5 For Half yearly returns. you have to mention Emp Insurance Number and Employees Numbers and 6 Months salary amount amd amount of contribution and Esi Hospital Name of the Employees prefered etc..

you have to acknowledge return in form .05 in one copy for you and have to submit Quadruplicate copies to your local office before 11th nov.10 with all six challans. Regards Lalit Papnoi
