We are going to deposit labour welfare fund for our Kolkata office under West Bengal labour welfare fund. Could you pls guide me that Any Covering letter or form need to be filled along with it? Someone has told me to fill form D but still I do not have the format.

There is a new ammendment in Chattisgarh Labour welfare fund contribution rates with effect from Mar 2012
New rates are 15 Rs from Employee and 45 Rs from Employer from and Minimum Contribution is Rs 1500.
(earlier contributions were Rupees 6 employee and 18 from employeer)
* If Employer Share Is Less Than 1500, Then Minimum Rs. 1500 Has To Be Paid
